    I've submitted a bug for this, but there is a chance this is by design: if you take the door out and fall off the bridge, there is a chaos shrine below.
    Since it is a chaos shrine it will take you to a random place upon rezzing.

    It was due to me hitting the gate switch above which locked the door, then falling down into room, getting killed by the mobs that spawned, since exit was closed I couldn't leave room!
