Item Missing from Epitaph (spoiler)

4. After resetting my quests, I went to the Epitaph but the lens fragment was missing in the pool. I had found it before the reset.

6. Windows 10; GTX 1080 (forgot to /bug this - sorry)

 

I also went there and was not able to get a duplicate lens fragment.

I do not have the quest, but I was able to get a duplicate page of Humility - which is another quest item from the same quest path.

Details: There should be a quest item here, but it is not dropping now.
Steps to Reproduce: Try to get the quest item here. I was able to get the Page of Humility twice today from the same quest series.
