Details: i scrolled out of the crypt of the avatar (got the info to continue and got miles' quest but couldn't turn it in) and still have this item. it doesn't work anymore, fortunately.

This is how the item is supposed to work. The summon whistle will only work for short-term companions in the scene they are obtained in. When you progress the main Path of Love quest (which is seperate from Miles' quest to calm Eris) within the Crypt of the Avatar the summon whistle will despawn. You will need to speak to Eris to get to that point. Eris will progress the main quest whether or not she is calmed by Miles' request.

 

i had already previously completed the eris side.

 

No, if you had Samael would be communicative in Spite and you no longer would have the summon whistle. You need to speak with Samael, after both NPCs instruct as well as the journal entry, to finish the Crypt sequence with him.

 

no, really. i did the main midras quest months ago, but had not yet done spite. i only came back to try the new one you added. did it lose the previous flag?
