Reassemble Vermillion Lens Quest Appeared in Journal - Have not completed the Truth/Love/Courage

I zoned in to Crooked Shank, and this quest called "Reassemble the Vermillion Lens" just appeared in my quest journal. I asked around, and have learned it shouldn't appear until I have completed all 3 of the Truth/Love/Courage storylines... which I have not done.

Details: I have completed the Path of Truth quests, and am working my way through the Paths of Love and Courage. Suddenly there is a new quest in my journal when I zoned in to Crooked Shank - Reassemble the Vermillion Lens. It is clearly a mid-quest entry, but I have never seen anything like it before, so I asked a hospitaller about it and was told it should not appear until I have completed all 3 of the Path quests. I don't know what caused it or where it came from.

Steps to Reproduce: Zone in to Crooked Shank???
