Details: Rung ladder: If clicked on upper part, intending to climb down, the screen turns black for a moment as if climbing but one stays in the same place above the ladder.

When used from below the ladder works normally and takes one up.

as designed. ladders placed for getting out of water generally only have one trigger direction. one does not expect to use a ladder to get INTO the water, you can just jump in.

 

That ladder is placed near the pipe that can be used to traverse the water to get to the Moontower. I tried to use it not to get in the water but assuming that I would get down to the pipe to walk over it to the Tower.

 

I just remembered an exception to the general rule: The climbable rope / ladder in Elad´s Lighthouse at the Cliffs of Insanity: That can be used both to get down and to get up again. I guess the difference is the extreme height of the cliff where people might shy away from simply jumping into the water... ( not knowing that jumping into water does not cause the same damage as a jump from a large height on land ).
