I use "E" a lot to avoid double click fatigue. It seems that multiple times per day I end up taking everything from my chests. The most typical is when I am sorting through multiple chests. I will mouseover one chest, press "E" and mouse over the next chest and press "E", but instead of opening the next chest, the first chest dumps into my bag and closes.

I know that I can avoid this, but for the life of me I can't stop screwing this up. Open and take all are not created equal.

After press 'E', Please add a confirmation dialog before taking all items from the chest or trunk.

"E" was created to lessen the double click effort and to make looting quicker, especially when fighting mobs.
Now you want "E" and a click?

"E" and a click only for Containers like Chests and Trunks which are for storage at home and with a lot of items only.

A confirmation dialog might work if you're taking a high quantity of items, or, a moderate quantity of items that add up to a certain weight

Hmmm... these personal experiences are weird. For me a "confirmation" dialog would just clump my interface even more (I don't usually experience any of these) but i guess an "option" to display this checkbox for people who need it might work.

How about just making it contextual? "E" loots everything unless the chest is owned by a player (including your own).

In my case, most of this caused because the same key is used to both open chests and take all items. If I am mousing over another chest, there is a 99.9% chance I want to open that chest. I really never want to take all from my own chests. If I did, I would actually select all the items and drag them.

As it is now, instead of saving me from double clicking, I have to wade through my backpack dragging the items back... a whole lot of clicks.

Even with some care, I still manage to do this once or twice a week.

If you do not click away from the backpack, all the items you just moved are still selected.
Just double click the former container, and drag them back in.

True if you have the backpack open. Many times I am just looking through chests and my backpack is closed. Thanks for the tip though. ;)

While this is technically correct, if you have auto-stacking on then it's often not really useful.
