Encounter issue

Title: Encounter issue
Reproduction Rate: 10% -40% (sorry I cannot be more accurate than this)
Blocker? Potentially but not likely
Details: Defeat/complete/run out of an encounter, automatically zone into another encounter that spawns in the same exact spot upon exiting and get pulled into new encounter. This is not a new bug but it is still a bug and I did experience it on QA.

Here is another one just a few minutes later in a different spot. Zoned out and it was spawned on top of me as I exited. This is a common thing.
