    Details: I got the shard of the lens at this location, but the journal entry doesn't clear - and there is no journal entry or hint pointing you to go to Oracle Colossus for the last piece (had to ask a friend where to go..)

    If you read the Ancient Obsidian Dispatch with the fragment, it clears that entry and says reassemble the Vermillion Lens. It also says to go to Oracle Colossus and Epitaph.
