[Known] Discovered locations, rediscovered, repeatedly

1. Reproduction Rate: 100%
2. Blocker? no, just very very annoying.
3. Details: Every time I discover a new location or point of interest
   on the world map or within a scene, it says it added it to my map.
   However, every time I happen to run by that place again, it adds it to my
   map yet again.
4. Steps to Reproduce: Walk by location discovered and added to map. walk by it again
   adds it to the map. I noticed its especially noticeable after logging in again.
5. User Specs: Ryzen 7 1700, 16GB RAM, RX580 8GB, samsung 850 pro 512GB 50% capacity.
   Wacom Cintiq Companion 2, 16GB, Intel IRIS 6600, 512SSD m.2 512GB.
   
   Both machines reproduce the same error.
