Most "exciting" quest yet - *yawn*...

    My opinion is that all quests should throw the questor into single player mode for the duration of the quest. Thus the game can keep track of progress and instances reflect the results of actions accomplished. If the player wishes to leave the quest midway, then he can return to multiplayer, friends, or whatever mode (perhaps with a toggle) and rejoin the quest later where he left off. All quest beginning NPC would always be seen in the mmo non-quest mode so everyone sees the same thing unless they are in a quest mode.

    Still this does not solve the problem of cities not being able to create an instance. If you would have a quest in a city in which you have to catch a murderer, he will never go to jail or he will never be hanged as an instance, in which he is in jail or dead cannot be created. So he will never be punished for his crimes. There won't be consequences for our actions and choices.
