Phantom Death in the Fall

While fighting on the highest level of the fall, in the room with skeletons and ancient obsidian golem, I got killed by nothing. There was no player or monster present (aside from my team member).

There is no record in the chat journal. Each time I took damage, it got progressively higher, until eventually it killed me in one hit (with more than 1,000 hp). It also seemed to occur at regular intervals, several seconds apart.

This has happened to me before in the Fall, but I assumed it was some kind of an anomaly (Vruul room, fighting two earth elemental, took several big hits that didn't show up in the journal until it killed me).
